Eye Examinations

Children enrolling in kindergarten and children enrolling in a public, private, or parochial school in Illinois for the first time, must receive an eye exam before October 15^th of the school year.  The examination must be completed by a physician licensed to practice medicine in all of its branches or a licensed optometrist and recorded on the State Vision Form.  The vision exam must be performed within the previous year.  This is a requirement of Public Act 095-0671.
